NHTD Service Coordinator/Health Homes Care Manager – Full Time

RCAL is seeking a full time NHTD Service Coordinator/Health Homes Care Manager. Must the following qualifications:

  • Bachelor’s Degree in Human Services and 2 years of experience providing service coordination, or a related field such as Registered Nurse (RN)
  • At least one year experience working with people, or one year experience as a service coordinator/case manager with any population
  • Knowledge of disability rights, community resources, PCP, HCBS Waiver and related services
  • Outstanding organizational and interpersonal skills
  • Ability to network with other community services and resources
  • Excellent interpersonal, problem-solving, writing, and public speaking skills

Candidates must have the above qualifications and, at a minimum, two years of experience providing service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities. Alternatively, a minimum of a high school diploma or equivalent plus three years of relevant work experience is required.


  • Knowledge of community resources and services, legal and civil rights
  • Provide information and assistance with advocacy and self-advocacy as necessary
  • Complete required NHTD Waiver documentation in timely manner. This includes, but is not limited to, Provider Selection forms, NODs, PRI & Screens, ISRs, contact notes and face to face documentation
  • Complete all necessary Waiver training as defined in the NHTD Waiver Vendor Manual
  • Meet with consumers face to face at least once per month and document visit

Our Employment Specialists offer a variety of programs to strengthen a person’s skills and guide them through the career development process. Interested candidates must have experience supporting people with disabilities and the following qualifications:

  • High School Diploma or Associate’s Degree (preferred)
  • Minimum of one year of experience supporting with people with disabilities
  • A valid driver’s license and reliable personal transportation
  • Ability to work as part of a team
  • Positive attitude
  • Strong, creative and persuasive interpersonal skills
  • Ability to be empathetic
  • Strong work ethic


  • Teach vocational skills
  • Provide on-site support coaching in a manner to promote independent work
  • Complete paperwork as directed by Employment Coordinator
  • Provide information and assistance with advocacy and self-advocacy as necessary
  • Participate in regular staffing of office telephone providing inquiries and information
  • Accurate and timely record keeping and reporting
